Areas of assistance that will be provided by the coordinating nurse:

■ Management of home visits routine, as needed. ■ Assistance in coordinating rehabilitation and mental health therapy, and welfare services, according to need. ■ Assistance in scheduling appointments at clinics and attending these visits, as needed. ■ Guidance on ordering medications for home delivery. ■ Performing blood tests and other tests. ■ Coordinating paramedical and supplementary treatments, as needed. ■ Guidance, counselling, and support in additional areas and other treatments, as needed. Counselling and emotional therapy: Prior to your discharge from hospital, and to maintain continuous care, your social worker at the hospital and the coordinating nurse responsible for coordinating your treatment on behalf of the kupat cholim will ensure the arrangement of emotional therapy that suits your preferences. We recommend that you persevere with emotional therapy provided by trauma-specialized therapists. These therapists are available through the NII, kupot cholim, hospitals, and resilience centers. If you experience symptoms that significantly disrupt your daily life, please contact the nurse responsible for coordinating community care so that she can assist in scheduling an appointment at a multidisciplinary clinic of a hospital or kupat cholim. These clinics are staffed by professionals in the fields of psychology, psychiatry, and social work.
