Ministry of Health - Letter to the Returnees With your return, the healthcare system is making every effort to provide you and your family with the necessary treatment and support, while allowing space to enable acclimation, organization, and restoring a sense of control, according to your preferences. Supporting returnees in hospital from admission through discharge into the community: From the moment you arrive at the hospital and afterwards, we are here to ensure that you receive a comprehensive and continuous response for your physical, emotional, and mental needs. To this end, all professionals in the healthcare system are prepared for your admission to hospitals, community clinics, and all other health institutions. In this letter, we have gathered information regarding the care and support that you and your family will receive after your discharge from hospital. It should be emphasized that the care will be adapted to your choices, preferences, and according to the pace that is right for you. A coordinating community nurse on behalf of the kupat cholim: To provide you and your family with a single address for all medical needs (physical and mental), your kupat cholim has appointed a personal nurse to coordinate care. Her role is to adapt the care to your needs and preferences and, as far as possible, to enable the provision of care where you live. As needed, the nurse will involve an additional care giver, including home visits by specialist clinical nurses and physicians specializing in various fields. The nurse is an integral part of the package provided to you, which includes: a family case manager, team leader, social worker from the social services office, and a NII social worker.
